At SLM Services, our licensed nurses provide the following in home medical care services 

Home health Care – Home health services provided to adults, seniors, and pediatric clients who are recovering after a hospital or facility visit.  These services include short-term nursing, rehabilitation and assistive home health care. 

Pediatric Care – medical care services provided to patients under the age of 18.  Our services include, but are not limited to, companion care, wound care, medication administration.

Private Duty Nursing – Provides long term medical care in patients home.

Care Management – Conduct in-home visits to ensure patients care requirements are being followed after facility stay.  The services can include: medication reconciliations, diet monitoring, communication with medical team to confirm care regimens.
